MediCal insurance coverage started in one county; can services be moved to other county w/out loosing coverage?

My mom was diagnosed with a sudden serious illness and had brain surgery to remove an aneurysm at Riverside county hospital. Since she's under 65 but now considered indigent and disabled for at least 12 months, she was referred for Medi-Cal insurance. My question is, would it be possible to request a transfer referal for her to complete her outpatient treatment out of Sacramento County without loosing her coverage? Reason why is that we live in Sacramento county and it would be very easy to help her by bringing her home with us.
